WorkForms question types

 

WorkForms has all the elements you could ever need to build your perfect form. This includes a wide variety of question types to ensure your form gathers all the information you require from submitters!

Available question types

The question type you choose for your form will also correlate to a monday.com column type, where answers will filter into once a form is submitted. 

See each question type and the column type that it will correspond to on your monday.com board!

 

The mapping between your columns and question types is a great way of ensuring that the answers from your form responses are collated in a way that is easy to work with.

Add conditional questions

Conditional questions allow you to display certain follow-up questions in response to a specific answer that the WorkForm submitter provided. This is a must for creating more in-depth surveys or questionnaires.

To create conditional questions on your WorkForm, begin by clicking on a single or multi-select question that you'd like to create a condition for and select "Include condition". From there, choose one or more responses and select "Add new question" to add the follow-up question.

You can also customize the follow-up question to allow for further choices by making the question a multi-select question.

 

Tip: You can continue adding multiple conditions for the same question to really build out your form in the way that you wish!

 

Additionally, you can add multiple different conditions under each separate question by clicking "Add another condition" so that every separate response will lead to its own follow-up question! Take a look at the form below that contains multiple conditions depending on your answer to the "Which book are your requesting?" question.

 

Modify your questions

There are a variety of ways to modify your question settings, giving you full control over the way in which your questions will appear. These settings include which questions appear on the WorkForm and which will remain hidden, as well as other settings, such as whether they will be required to fill out, or not.

  • Hide or display questions

To customize the display of a question, click on a question block and select the "cog" icon in the bottom left-hand corner. Use the toggle next to "Hide question" to hide or display the question on the form. When the question is hidden, you'll see an "eye" icon with a slash through it appear on the top right-hand corner of the question block.

  • Make a question required 

You can also click on the "cog" icon to change the settings of a question to "Required", which will necessitate an answer from submitters in order to complete the form. When a question is required, a red Asterix will appear in the top right-hand corner of the question.

Tip: Any change that you make to your WorkForm will be saved automatically! Simply make the edits of your choice and you can pick up right where you left off next time you access your WorkForm.

 

  • Duplicate a question

The "Duplicate" button allows form creators to duplicate questions easily while creating a form. This can save time when creating similar questions in long forms. To duplicate a question, click on the question box and select the "Duplicate" icon located next to the "Trash" icon.

  • Column Mapping 

The column mapping icon will appear in the top right-hand corner of a question only if the question name does not match any of the column names on your board. When this icon appears, you can hover over the icon to see which column the information will be collated in. If the question and your column share the same name, the icon will not appear.

Default question settings

Certain question types have further modification settings that allow you to choose a default answer, making the submission experience more tailored toward your needs. These include the Date, Location, Phone, and True or false questions. 

  • Default date and timestamp

You can automate a date and timestamp of a date question by selecting the toggles next to "Today as default date", and "Include time". These will give you a clear indication of the date and time an answer was submitted.

  • Default phone prefix

When adding the Phone question to your form, you can predefine the dialing code prefix if you would only like to receive phone numbers from a specific region. You can also have an "Auto-fill country prefix" which allows submitters to choose the country dialing code from a dropdown list.

  • Default location 

With the Location question, you can select the option to "Auto-fill location" which will determine the submitters location automatically.

  • Default check-box

When selecting a True or false question type, you can choose to auto-fill the checkbox on the form so that the submitter is not required to manually do so themselves. This is useful for agreeing to terms and conditions or for enabling marketing content.

  • Pre-fill value

You have the option of setting a "Pre-fill value" for text column types which will auto-populate information when submitters are filling out their forms. Having pre-populated form fields gives you a more personalized and faster submission experience. 

To set this up, select the toggle next to "Pre-fill value" in the question settings, and type in the "Parameter" value you would like to auto-populate from the window that appears.

Once the form is sent to submitters, the question will be auto-populated with the answer, ensuring that the submitter does not need to fill in that question independently. 

 

Note: The "URL parameter" pre-fill option is only available to Enterprise and Pro accounts. 

 

The "URL parameter" can also be individualized by adding your required text at the end of the URL before sharing it. For instance, if you wish to send the form to someone specific you can have the URL parameter pre-populate the person's name by adding their name to the end of the URL:

Tip: If you are sending out multiple individualized forms, you can use a mailing system such as Mailchimp to automate the individualization of the URL parameter. 

 

There are two "Source" types to choose from when using the Pre-fill value; "Account" or "URL parameter". If the submitter is a member of your monday.com account, the "Account" option will allow pre-fill form fields to be auto-populated with monday account information. You can then select which "Field" of information you would like the question to pre-fill.

Note: The "Account" pre-fill option is only available to Enterprise accounts. 

 

Single and Multi-Select questions

The single select or multi select questions allows the submitter to select either one option or multiple options from a list. You can easily add different options to the question, reorder the options by dragging and dropping them, as well as hide certain options from the submitter.

By clicking on "Question settings" in the bottom left-hand corner of the question box, you can choose to display your single or multi select options either as a list, dropdown or horizontally.

The options provided in the question can be sorted by customized order, alphabetically, or randomized. To sort the option, click on the dropdown arrow below "Options order", and choose from the dropdown menu how you would like to sort the options.

From within the questions settings, you can also choose how many options a submitter can select within the Multi-select question. To choose a limit, toggle on next to "Limit option selection", and select the amount of selectable options from the dropdown list.

People Column question

Wanting to assign an item to a specific person using WorkForms without having to create an automation? Not a problem. You can add a people column question to your form! The people column enables the form submitter to specify which member of the account is related to the submission and is very useful for any request type use cases.

Note: It is not possible to assign a team in WorkForms. The People Column question is only for individual person selection, and is only available if the form is restricted to account members only. Read more about WorkForm account level permissions.

 

Subitems Question 

By utilizing a Subitems question type in your form, you can keep more detailed information from the form submission clearly mapped out on your connected board. The Subitems question will filter information from that question into the subitem section of the item on your board. Each form can contain no more than one Subitem question.

Note: The Subitem question type is currently available for Pro and Enterprise accounts. 

 

There are two ways in which to set up a Subitem question on your form. You can add the question type by creating the relevant columns and status labels directly in the subitem level on your board, after which a Subitem question will automatically be added to your form.

Note: Currently, the File, Signature, and Connect Boards question types are not supported in the Subitems question type.

 

Alternatively, you can add the Subitem question directly to your form, via the form builder. Once added, return to your board to configure the columns and labels in the subitem level to suit the needs of the question.

Column customization and configuration can only be done from within your board, and not via the form. This includes, adding/removing columns, customizing labels, ordering, and setting limits.
