Available question types
The question type you choose for your form will also correlate to a monday.com column type, where answers will filter into once a form is submitted.

The mapping between your columns and question types is a great way to ensure that answers from your form responses are collated in a way that is easy to work with.
Add conditional questions
Conditional logic in your questions allows you to consider what follow-up questions and information might be relevant once a submitter has answered a specific question in your WorkForm. This setting enables you, as the form creator, to display questions on the WorkForm only if the submitter's answers in a previous question meet a certain condition.
Modify your questions
You can modify your question settings in various ways, giving you full control over how your questions appear. These settings include which questions appear on the WorkForm and which remain hidden, as well as other settings, such as whether they are required to be filled out.
- Hide or display questions
To customize the display of a question, click on a question block and select the "cog" icon in the bottom left-hand corner. An editing menu will appear on the right-hand side of the screen. Use the toggle next to Hide question to hide or display the question on the form.
When the question is hidden, you'll see an "eye" icon with a slash through it appear in the top-right corner of the question block.
You can also choose to hide or display hidden content from your form and within the Question outline. To do this, toggle on or off Show hidden content at the bottom of the Question outline.
- Make a question required
You can also click the cog icon to set a question to Required, which will require submitters to provide an answer to complete the form. When a question is required, a red asterisk will appear in the top right-hand corner of the question.
- Duplicate a question
The Duplicate button enables form creators to easily duplicate questions while creating a form. This can save time when creating similar questions in long forms. To duplicate a question, click on the question box and select the Duplicate icon located next to the Trash icon.
- Column Mapping
The column mapping icon will appear in the top right-hand corner of a question only if the question name does not match any of the column names on your board. When this icon appears, you can hover over the icon to see which column the information will be collated in. If the question and your column share the same name, the icon will not appear.
Default question settings
Certain question types offer additional modification settings that let you choose a default answer, tailoring the submission experience to your needs. These include the Date, Location, Phone, and True/False questions.
- Default date and timestamp
You can automate the date and timestamp of a date question by selecting the toggles next to Today as default date and Include time. These will give you a clear indication of the date and time an answer was submitted.
- Default phone prefix
When adding the Phone question to your form, you can predefine the dialing code prefix if you would only like to receive phone numbers from a specific region. You can also enable Auto-fill country prefix, which lets submitters choose the country dialing code from a dropdown list.
- Default location
In the Location question, select Auto-fill location to automatically determine the submitter's location.
- Default check-box
When selecting a True or false question type, you can choose to auto-fill the checkbox on the form so that the submitter is not required to do so manually. This is useful for agreeing to terms and conditions or for enabling marketing content.
- Pre-fill value
You can set a Pre-fill value for certain column types, which will auto-populate information when submitters fill out their forms. Having pre-populated form fields gives you a more personalized and faster submission experience.
There are two Source types to choose from when using the Pre-fill value: Account or URL parameter. If the submitter is a member of your monday.com account, the "Account" option will auto-populate form fields with monday.com account information. You can then select which Field of information you would like the question to pre-fill.
The URL parameter can be used externally to your account and pulls data based on the URL configuration you send. To set this up, select the toggle next to Pre-fill value in the question settings, type the Parameter name you would like to auto-populate in the window that appears, and add the value to the URL.
Once the form is submitted, the question will be auto-populated with the answer, so the submitter does not need to fill it in independently.
Link new questions to existing columns
Adding a column to your board will not automatically create a question in your WorkForm. To connect a new question with an existing column on your board, you must search for the column name within the + Add content menu in your form.
When searching for a question type, it shows two options if an existing, unsynced column is available. By selecting the question option under Columns in, the question on your form will be paired with an existing column.
If you select the question option that is not associated with a column on your board, then a new column will be added to your board to gather the responses to this question.
Single and Multi-Select questions
Single-select or multi-select questions allow the submitter to select either one or multiple options from a list. You can easily add different options to the question, reorder them by dragging and dropping, and hide certain options from the submitter.
By clicking Question settings in the bottom left-hand corner of the question box, you can choose to display your single- or multi-select options as a list, a dropdown, or horizontally.
The options provided in the question can be sorted in a custom order, alphabetically, or randomly. To sort the options, click the dropdown arrow below Options order, then choose how you would like to sort them from the dropdown menu.
From within the question settings, you can also choose how many options a submitter can select within the Multi-select question. To choose a limit, toggle the Limit option selection on, and select the number of selectable options from the dropdown list.
People Column question
Wanting to assign an item to a specific person using WorkForms without having to create an automation? Not a problem. You can add a people column question to your form! The people column enables the form submitter to specify which account member is associated with the submission, and it is very useful for request-type use cases.
Connect Boards question
With our monday Connect Boards question, you can connect form submissions to your other boards on monday. This helps with connectivity and transparency!
To add this question to your WorkForm, first ensure the Connect Boards Column is added and configured in your board. The Connect Boards question will then be added to your form builder.
To activate this question, select Require login to monday.com in your Share form settings to restrict access to your account members only. Then, make sure the question is visible on your form by toggling off the Hide option in the question's settings.
If your Connect Boards Column is linked to one board, the form displays a single dropdown with that board's item names. If it's linked to multiple boards, the form shows two dropdowns: one to select the board and another to choose the item on that board.
Subitems question
By using a Subitems question type in your form, you can keep more detailed information from form submissions clearly mapped out on your connected board. The Subitems question will filter information from that question into the subitem section of the item on your board. Each form can contain no more than one Subitem question.
There are two ways to set up a Subitem question on your form. You can add the question type by creating the relevant columns and status labels directly in the subitem level on your board, after which a Subitem question will automatically be added to your form.
Alternatively, you can add the Subitem question directly to your form via the form builder. Once added, return to your board to configure the columns and labels in the subitem level to suit the needs of the question.
Column customization and configuration can only be done from within your board, and not via the form. This includes adding/removing columns, customizing labels, ordering, and setting limits.
Name and Update questions
You can choose to add the Name and Update questions to your WorkForm from the + Add content menu. This helps align incoming submissions with your board's structure and workflow.
The Name question will include the board's first column within the form, which is commonly used to capture item names.
The Update question allows you to collect free-form text, which will be added to the updates section of an item.
