Designed to be an easy and efficient way to build forms, to ensure all of your needs are met. From the design, layout, and question choices, with monday.com WorkForms, you can create the perfect form with a few simple clicks 🖱️

 

Introduce your WorkForm

Within the "Edit" tab at the top of your form, you can tailor the Welcome page, Logo, Title, Description, and Submission button to align with your brand and clearly display the form's purpose.

  • Welcome page

You can choose to add and customize a welcome page on your form for submitters to begin their journey in completing your WorkForm. Click the toggle at the top of your form to begin customizing the Title, Description, and Custom start button.

  • Add a logo

To add a logo, hover over the title block on your form, and click on the "Add logo" button in the top left-hand corner. A window will open, allowing you to select a file from your computer.

Once you have selected your image, you can drag the squares to crop your image to a size that suits your needs. 

 

Note: To optimize the quality of your form logo, we recommend using a file with a max size of 4 MB and 100 pixels, which will maintain the image resolution.

 

  • Add a Title and Description 

The title and description on your WorkForm are a great way to explain what the form is used for or include any additional details that you may need the responder to know. To add or edit the title and description of your form, simply click on the text box and type out your desired text.

Note: Form description areas, such as the Welcome page, form description, question description, and Thank you description, all support rich text formatting. This allows you to use formatting features like bold, italic, bullet points, hyperlinks, JPG/PNG images, and more! 

 

  • Header alignment 

You can choose to align your form header either to match the alignment of your form or independently to the left, right, and center of the form. To choose your header alignment, hover over the header block and click on "Settings". From here, choose your alignment via the dropdown menu.

Note: From within the same "Settings" options, you can add "Alt text". This text appears in place of your logo in the event that the image fails to load on a user's screen and helps screen-reading tools describe images to visually impaired readers.

 

  • Submit button

In the "Submission button" block at the bottom of your form, you can customize the submission button with the text of your choice! Simply click on the submission block and begin customizing the button text in the text box.

  • Thank you screen

In the "Thank you" block at the bottom of your form, you can customize the submission's title and description! You can also choose whether to hide the success image.

Design your WorkForm

With WorkForms, designing the form of your dreams is easy! The build tools to the right of your form will help you give it the look and feel you need

  • Add a background color or image

To modify the background color or image of your WorkForm, click on the image icon on the right-hand side of your screen. In the "Color" tab, you can choose a solid color for your background, and in the "Image" tab, you'll find some of our favorite background images to choose from. Alternatively, you can also upload your own image by clicking "+ Upload your own" under the "Image tab".

Tip: There is no specific ratio or size requirement for a background image. However, we recommend keeping your image as small as possible to receive a faster loading time.

 

  • Choose the format of your WorkForm

When setting up your WorkForm, you can choose whether you'd like your form questions presented in "classic" format, or if you'd like the "one by one" format.

With the classic format, all the questions in your WorkForm will appear in one view. The form responder can see all the questions at once and simply scroll down as they complete the questions within the form.

The one-by-one format allows the questions from your form to appear individually while the form is being filled out, eliminating the need to scroll and navigate between different questions. Using this formatting type, the form submitter can focus on each question without being overwhelmed by the entire form.

Tip: The one-by-one format is great for feedback surveys, contact sales, forms, requests, registrations, and more!

 

  • Adjust the alignment of your WorkForm

Clicking on the grid icon on the right side of your screen will allow you to adjust the placement of your WorkForm amongst its background. You can choose to align the form to the left, right, or center, and whether you'd like a frame surrounding it.

  • Edit text color and font

You can change the color and font of the text displayed on your WorkForm by clicking on the "T" icon.

  • Form and Text Direction

WorkForms also offer support for RTL (right-to-left) languages. If you'd like to change your form text to go from right to left, you can do so by clicking on the button with the two arrows on the far right of your screen.

Choose your question type

WorkForms gives you a wide variety of question types to choose from, ensuring you have flexibility when it comes to building your form to suit your needs.

See each question type and the column type that it will correspond to on your monday.com board!

 

  • Add question and answer choices

It's time to add and customize your WorkForm questions! To begin, select "Add new question" and choose the type of question you want.

Hovering over each of the blocks within the question box will allow you to edit the question title and the question's optional description. If you've chosen a question type that requires pre-set response options, such as a single-select, multi-select, or True/False question, you can click into the options to edit them as you wish.

The question type you choose here will dictate the type of column that will be added to your board if you connect your WorkForm to a monday.com account.

 

  • Modify questions

You control which questions appear on the WorkForm and which remain hidden, as well as various other question settings. By clicking on the "cog" icon, you will see further settings available for that question. These can include making a question "Required", setting the default date, and providing a timestamp.

The questions outline

Using the questions outline in the left-hand pane of your WorkForm, you can see all of your form questions in list view, navigate to each question with a click, and easily drag and drop questions to re-order them!

  • Open the questions outline

To open the questions outline, click on the "list" icon at the top left corner of your WorkForm. Once selected, the outline will open as a left-hand pane on your WorkForm editor.

To collapse the question outline, click on the "arrow" icon at the top of your question outline, next to "Content". The left-hand pane will then be hidden. 

 

  • Navigate

No need to scroll through your WorkForm to find a specific question; you can navigate to a specific question using the question outline. Simply click on the question in the left-hand pane, and your screen will jump directly to that question.

  • Re-Order questions

Want to reorder your questions in your WorkForm? To move a question, click on the "dots" icon to the right of the question name in your question outline and drag the question to the spot in the list where you would like to move it.

Questions that have been "Hidden" on your WorkForm will appear greyed out in the question outline. 

 

Note: The drag-and-drop feature in the questions outline does not support moving conditional questions. If you try to drag and drop a conditional question, it will return to its original position. 

 

Preview your WorkForm 

Now that all of your design elements have come together, you can preview your WorkForm to see how it will look to submitters! Click on the "Preview" button in the top left-hand corner of your form. A new browser tab will open, showing you the preview of your form.
